Separation Agreement Provision

1. Coverage for Spouse and Children. Husband will maintain the medical, dental, and disability insurance currently in effect and provided through his employer for the benefit of Wife and the Children of their marriage. If such insurance coverage terminates, Husband will immediately provide substantially equivalent coverage.

2. Termination of Coverage.

a. Spouses' Benefits. Husband's obligation to provide medical, dental, and disability coverage for Wife will terminate upon divorce [optional clause: or upon Wife's remarriage or cohabitation with a person with whom she is meretriciously involved].

b. Children's Benefits. Husband's obligation to provide medical, dental, and disability coverage for the parties' children will terminate when each child ceases to be eligible for such coverage, when he or she marries or dies, or when he or she enters into military service, whichever occurs first.

3. Noninsured Medical Expenses. Medical, dental, or other expenses resulting from a child's injury or illness that are not covered by Husband's insurance will be paid by [Wife or Husband]. [Optional clause: but such obligation shall not exceed ---------- dollars ($----------) per year.]
